You must be a Washington resident. There is no age requirement, however, patients under the age of 18 will need a parent or guardian to register as their caregiver.

The next step after being certified by a doctor is to apply with the Washington State Department of Health as a medical patient. In order to apply, patients will need to have:

You will need a valid photo ID and if possible, at least one form of proof of your medical condition such as medical records, pill bottles, office visit summaries, or a letter from your doctor.

The initial fee to be evaluated by our provider is $199 payable by credit or debit. The fee includes the visit with the provider, help with the state application, and any other assistance needed throughout the process. There is also an ID card fee that typically ranges from $1-10.
