You will need a valid photo ID and if possible, at least one form of proof of your medical condition such as medical records, pill bottles, office visit summaries, or a letter from your doctor.

If you don't have medical documentation, you can still get certified! Call or text us at 240-356-1000 so that we can schedule you with a doctor that can diagnose and then a doctor that can certify you. We do not charge extra for the diagnosis.

Once your telemedicine appointment is booked, you'll receive a confirmation email shortly after. This email will have the link to your intake form which is where you will fill out your medical information as well as upload your ID and any medical documentation you are able to provide.

The initial fee to be evaluated by our doctor is $200 payable by credit or debit.  The fee includes the visit with the doctor, help with the state application, medical marijuana education, and any other assistance needed throughout the process.  The state application fee for your registry ID card is $150.

Only a licensed medical marijuana doctor can determine if you qualify for the program, so we cannot guarantee your approval. However, if you do not get approved by our doctor you will receive a FULL refund! An Arizona-licensed medical marijuana doctor will examine you, review any medical documentation you are able to provide, and assess your medical history. Appointments are typically about 10-15 minutes. We will also maintain your patient records and provide follow-up care as needed.

No. Doctors must hold a valid license to practice medicine in Arizona and register with the Department of Public Health in order to certify patients for a medical marijuana card. Yes. Children (under the age of 18) can become medical marijuana patients in Arizona. However, the child must have a parent or guardian register as his/her caregiver.

A caregiver is an Arizona resident at least 21 years of age who is designated to be able to purchase medical marijuana products from dispensaries on behalf of a qualified patient.

You’ll need to submit a medical cannabis patient application to the state online along with a valid photo ID, a current passport-style photo of yourself, medical marijuana written certification form issued by a doctor within the last 90 days, the required patient attestation form, and the state application fee of $150 or $200 if you're a caregiver.

If your application is approved by the state, you’ll receive your registry identification card within 10 business days via postal mail.  Once you have your card, you can immediately start enjoying the benefits of being a medical marijuana patient in Arizona.

No. Insurance companies do not cover any costs associated with medical marijuana due to conflicts with federal law.

Legal patients can obtain medical marijuana products from licensed dispensaries located throughout the state. Patients will also be able to use their card in other medical states that offer reciprocity.

As a medical cardholder, you may purchase and possess up to 2.5 ounces of medical marijuana every two weeks.  Recreational customers will be limited to just 1 ounce every two weeks.

Virtually all types of medical marijuana will be available at dispensaries in Arizona including raw flower, concentrates, edibles, tinctures, topicals, transdermal patches, and more.  Seeds, clones, and other growing products will also be available.

Yes.  Adults 21 or older may cultivate up to six (6) plants in their home.

No, as a medical patient you are protected by HIPAA. Green Health Docs makes the privacy and security of our patients’ confidential information a top priority. No one will be able to find out your status as a medical marijuana patient unless you openly tell them or if you are administered a drug test for marijuana.

Yes. With Arizona being an at-will state and there not currently being any legislation in place that protects medical marijuana patients in the workplace, medical cardholders can still be administered drug screenings and receive disciplinary action regarding the results of drug screenings. For this reason, we recommend making sure you are fully aware of your employer’s policy regarding drug screenings and medical marijuana before moving forward with the process.

Arizona does not have any restrictions regarding patients on probation or parole, however, you must get permission from your probation or parole officer first before pursuing a medical marijuana card.

As a legal medical marijuana patient, you are protected. Patients do not have to disclose that they possess medical marijuana and do not have to consent to a search. However, if a search is conducted and medical marijuana is found, the patient should immediately present their patient ID card or direct law enforcement to the state database to show that they are a legal patient.

Yes, there are currently multiple states that accept Arizona medical marijuana cards.  Because reciprocity laws differ from state to state and with each dispensary, we recommend contacting a dispensary in the state you’re interested in visiting to find out if they will accept your card.

You are required to be re-certified by a doctor once every two (2) years; the cost to renew is $200.  You will also need to renew your card with the state every two (2) years for $150.

Yes! You do not have to be seen by the same doctor each year. If you were certified by another medical marijuana doctor last year and would like to switch to Green Health Docs, we will be happy to take care of you!
