Phoenix Medical Marijuana Card Doctors

With Arizona medical marijuana dispensaries now open, qualified Phoenix residents now have access to medical marijuana. In order to purchase medical marijuana at state-licensed dispensaries, patients must be seen by a doctor approved by the state of AZ. At Green Health Docs, we like to make things simple for you. Getting an Arizona medical marijuana card is a straightforward process, to qualify you must:

(1) Be a resident of the state of Arizona. There is no age restriction. Patients under the age of 18 will require a parent or guardian to register as their caregiver.

(2) Be diagnosed with one of the state-approved qualifying medical conditions. You will need to have at least one form of medical proof of your diagnosis such as a prescription bottle, office visit summary, or a letter from your doctor.

(3) Be evaluated by an Arizona state-approved medical marijuana doctor. Each doctor at Green Health Docs is registered with the state and can certify you for a medical marijuana card. Once approved, we'll issue your physician written certification the same day.

(4) Register with the Arizona Department of Health. Our doctor will issue your written certification form on the same day. You will then have 90 days to submit your application to the state online along with the required documentation and $150 application fee. If your application is accepted, you will then receive your physical card in the mail within about 10 business days. Your card is valid for two (2) years from the date it was issued, and you should renew your card at least 30 days prior to the expiration date.
